They might be at home awaiting a placement, and they and their families need some interim support. Or they might have returned home and are climatising to a new environment.

During this period, we deploy our specialist support staff to help individuals with various tasks and guide them to access valuable community resources.

Our outreach support not only helps service users but also provides peace of mind for family members and carers, helping them adapt to the transition period, too.

Outreach support is offered on a case-by-case basis, balancing the needs of the service user and their family with what our team can offer.
